Something wiped my template information (in the Settings page)
Does anyone have a sample that I could use?

John, I can retrieve a version of your template if you compress your backup folder from your SysnativeBSODApps directory. Upload backup.zip here.
 
Thanks Mike! Could you let me know how - that way I'll be able to do it on my own later on.
FWIW - I did read the manual (help file) but am a little confused on how it's structured.

Also, my SendTo context menu item went missing, so I had to do a workaround to compress it.
I have no idea, but this is Win10 and it changes with each update (and I did have an update the other day to build 10122)

The app saves backup files every time a user saves and runs it after making changes.
  1. Start the apps.
  2. Click Change Settings
  3. Click File
  4. Click Open
  5. Navigate to your backup directory under SysnativeBSODApps
  6. Open 82.zdn to load your most recent backup with the template.txt intact

Alternatively, you can just replace the SysnativeBSODApps\dmpOptions\template.txt file with the file attached to this post.

Template issue is because I switched to Bleeping Computer!
Spotted one error mentioning that the field for the topic wasn't valid.

I'm trying to look up how to add Bleeping Computer to the allowed list,
In the meantime I'll either leave it blank or will put Sysnative in the field
