Perhaps the spam filter tagged the email because the email itself, the content, was identified as spam? And so the filter caught the spam in two ways - by the stinky content, and by the sender too. BTW, "SPAM" is a canned ham and pork product considered a delicacy in Hawaii. The word for malicious emails does not deserve being in full upper case letters. Spam, when used as the first word in a sentence, is tolerated, but only because proper grammar and sentence syntax demands it.

As you mentioned, something triggered COX to send it to the SPAM Folder. My question is still why it even showed up there if it was to be blocked.

Are you sure they are the same email, and not, perhaps where one copy was sent a few seconds later? Has this happened more than once?

Yeah, good luck with Cox support. That said, if this is just a one-time thing, I personally would not worry about it and dismiss it as a one-time fluke. But if it is happening often, then I would dig deeper.

That's one form of blocking however, that user can send another email your way until you, the user tells the filter to block it from being sent to you. This is at least how my Gmail works.
