I have it to emulate how XP roughly looked at. I have noticed that in my Program Folder there are a few icons that are blank. See picture below. I have tried right clicking the problem icons and changing the icon. A typical message is " Shortcut - Unable to save changes to 'PrintDialog.lnk -Access denied". I have checked the security tab on the icon properties and have all the permissions selected. Also after the Windows 10 Fall Update I noticed that some icons in my "normal" Windows Start menu are blank...specifically the ones under "Most Used". All other app icons are fine. See picture below. I switch back and forth between this start menu and the XP like one from classic shell. I have tried the icon cache rebuild and rebooted as per the link below a few times without luck: Icon Cache - Rebuild in Windows 10 - Windows 10 Forums I have also uninstalled classic shell as I thought maybe it was causing problems, but this had no effect on the "Most Used" icons. Any ideas? Any help would be appreciated.

Use your most used apps/programs and see if the problem is solved (i.e., if they'll appear with the correct icon).

I also found that more than half my apps would open for a fraction of second and then close. I haven't gone more than a few weeks of having something flake out, and me doing a refresh. I tried reinstalling Windows 10 built in apps as per the website below. It has worked for me before but not this time. https://thomas.vanhoutte.be/miniblog/reinstall-default-build-in-windows-10-apps/ I loaded up my Win10_1511_English_x64.iso from boot and started right from scratch wiping everything, and then reinstalled all my programs. My "show most used" apps icons and all my apps work correctly. I still have the same three icons (Devices Flow, Miracast PlayTo renderer, and Print Dialog) in my Program Folder that are blank after the reinstall as per my first picture. This is not a huge deal, but am curious why these icons are screwed up and if they can be fixed??

I also use Classic Shell, I have also noticed it on two different computers. My wife's brand new HP ENVY and my brand new Alienware 17r3. I want the icon issue fixed, I'm wondering why Microsoft hasn't fixed it yet. Don't tell me they haven't noticed. I know they are only 3 icons, but I like a nice clean looking system. :banghead:
