I have seen a video of this antenna in YouTube,

How is antenna connected to the isp system to access internet?

Does it need a router connected at other end of the wire?

At 1:25 into the video, you can see he connects an Ethernet cable to that device. And that device appears to be a WAP - wireless access point. So yes, the other end of the cable goes to a router, the router then connects to a modem and the modem connects to the ISP's network.
