Upgrade from Windows 8.1 to Windows 10

Hello, I am having trouble upgrading my Windows 8.1 to Windows 10 using the Media Creation Tool.
Every time I try to update I've gotten this error: 0x8007002C - 0x4000D "the installation failed in the SECOND_BOOT phase with an error during the MIGRATE_DATA operation.

I've tried to disabling startup apps and deleted my antivirus programs. Not sure what's causing it and I'm inexperienced with these sorts of things.
 
From SECOND_BOOT phase it's now SAFE_OS phase.
I've tried the following;
1. Disabling my antivirus (and deleting them)
2. Disabling and Clean Booting my non-microsoft apps
3. Command Prompt with SFC and DISM
4. Using ISO instead of the Media Tool.
5. Resetting Windows Update Components.
6. Disabling LAN + USB such as headsets and other drives.

What else could the problem be?

Were SFC/DISM clean?
From what I checked it's all clean:

For DISM, I've tried restoring health just to see if it would do anything yet nothing happened.
 
My setuperr is blank while my setupact is not. Just provided both in case any of the 2 will help.
There are other setuperr files on different subfolders too. Should I provide those as well?
 
Please try and locate rollbackinfo.ini

In addition to that, do the following:

Step#1 - FRST Scan

1. Please download Farbar Recovery Scan Tool and save it to your Desktop.
Note: You need to run the 64-bit Version so please ensure you download that one.
2. Right-click FRST64.exe and click Run as Administrator to run it as administrator. When the tool opens, click Yes to disclaimer.
3. Please ensure you place a check mark in the Addition.txt check box at the bottom of the form before running (if not already checked).
4. Press Scan button.
5. It will produce a log called FRST.txt in the same directory the tool is run from (which should now be the desktop)
6. Please attach the log back here.
7. Another log (Addition.txt - also located in the same directory as FRST64.exe) will be generated Please also attach that along with the FRST.txt in your reply.

Step#1 - FRST Fix
NOTICE: This script was written specifically for this user, for use on that particular machine. Running this on another machine may cause damage to your operating system
1. Please download Farbar Recovery Scan Tool and save it to your Desktop.
Note: You need to run the 64-bit Version so please ensure you download that one.
2. Download the attached fixlist.txt and save it to the Desktop.
Note. It's important that both files, FRST64 and fixlist.txt are in the same location or the fix will not work (in this case...the desktop).
3. Run FRST64 by Right-Clicking on the file and choosing Run as administrator.
4. Press the Fix button just once and wait. If for some reason the tool needs a restart, please make sure you let the system restart normally. After that let the tool complete its run.
5. When finished FRST64 will generate a log on the Desktop (Fixlog.txt). Please post the contents of it in your reply.

FRST Search
1. Click your Start button and type in cmd.
2.After you find the Command Prompt, right click on it and select Run as Administrator.
3. Copy and paste the following into the Command Prompt:

reg load HKLM\COMPONENTS C:\WINDOWS\SYSTEM32\CONFIG\COMPONENTS

4. Please download Farbar Recovery Scan Tool and save it to your Desktop.
Note: You need to run the 64-bit Version so please ensure you download that one.
5. Run FRST64 by Right-Clicking on the file and choosing Run as administrator.
6. Copy and paste SearchAll: AVG into the Search box and click the Search Files button.
7. When the scan is complete a notepad window will open with the results. Please attach this to your next reply. It is saved on your desktop named Search.txt.
