Windows Server 2016 Srd Wont Update - Second Server

Hello SysNative (you gurus you!),

I am making this post for the 2nd server I am having the same issue as this thread:
Literally the same exact issue, when you try to run Windows Updates since KB5048671... it will download the update, try to install it and then you go to reboot and it has to revert. On top of that, DISM just fails no matter if I attach a good ISO or not. Posting all relevant logs below:

  • Tried in-place upgrade to 2019 and 2022
    • Both fail with SAFE_OS error
      • 0xC1900101 - 0x20017
  • SFC scan results:
    • C:\Windows\system32>sfc /scannow

      Beginning system scan. This process will take some time.

      Beginning verification phase of system scan.
      Verification 100% complete.

      Windows Resource Protection found corrupt files but was unable to fix some
      of them. Details are included in the CBS.Log windir\Logs\CBS\CBS.log. For
      example C:\Windows\Logs\CBS\CBS.log. Note that logging is currently not
      supported in offline servicing scenarios.
  • DISM results:
    • PS C:\Windows\system32> DISM /Online /Cleanup-Image /RestoreHealth

      Deployment Image Servicing and Management tool

      The array bounds are invalid.

      The DISM log file can be found at C:\Windows\Logs\DISM\dism.log

Follow the instructions below please.

WARNING! The following fix is specific to the user's system in this thread only. No one else should follow these instructions, as it could damage your system.
  • Download
    myjIXnC.png
    SFCFix and move the executable to your desktop.
  • Download the attachment SFCFix.zip and save it on your desktop.
  • Save any work you have open, and close all programs.
  • Drag the SFCFix.zip file over the SFCFix.exe executable and release it.

  • SFCFix will launch, let it complete.
  • Once done, a file will appear on your desktop, called SFCFix.txt.
  • Attach that file to your next reply.

Restart the machine.
Run the command sfc /scannow in an elevated commandprompt and report the result.
If it fails, copy the file C:\Windows\Logs\CBS\cbs.log to your desktop.
Zip the copied file and attach the zipped cbs.log to your next reply.
 
So sfc scan completed successfully, found corruption and fixed it. Rebooted and ran updates but updates still failed. attaching logs and snippets of the update errors.

ChkDsk Scan
  • Right-click the Start button and click Commandprompt (Run as administrator).
  • A window appears. Typ CHKDSK and press the enter key.
  • Wait until CHKDSK is finished.
  • Download the attached ZIP file (ListChkdskResult.exe by SleepyDude), extract it and save the file ListChkdskResult.exe on your desktop.

  • Rightclick

    ListChkdskResult.exe and click Run as administrator.
  • On completion the tool will save a textfile called ListChkdskResult.txt on your desktop.
  • Attach this text file to your next reply.
 
No problem on the delay.

FRST File Search
  1. Right-click on the file FRST64.exe and choose Run as administrator.
  2. Copy and paste *KB4091664* into the Search box and click the Search Files button.
  3. When the scan is complete, a message will display that 'Search.txt' is saved in the same folder FRST was started from. Notepad will open this file also.
  4. Close Notepad and attach the file 'Search.txt' to your next reply.
