Windows 2012 R2 won't install updates due to KB4541509, but its not installed

Got a Windows 2012 R2 server that is fairly up to date (at least March 2020), but since then it won't update. I get stuck in a loop...

CBS Failed to resolve package 'Package_69_for_KB4541509~31bf3856ad364e35~amd64~~6.3.1.5' [HRESULT = 0x800f0831 - CBS_E_STORE_CORRUPTION]

BUT... KB4541509 is not installed.

I can install it, but that comes up with (almost) the same error. (Sometimes, it references a different Package_<number>, but always for KB4541509.

I've done/attempted to do many things at this point that I'm pulling out what hair I have left! Please help!

I've run the SFC /scannow, DISM restorehealth and SFCFix, they all come back clean. Or, at least they will until I try to do another update (either through DISM add-package or straight up Windows updates). Then, inevitably, the CBS log will get the error above (usually pointing to KB4541509).

I don't know if I need to actually get KB4541509 to successfully install (somehow?), or if I can get around it (with a superseded patch?)

I have attached some of the commands that I've run (just to give you an idea of what I'm doing).

Any help is (desperately) needed and appreciated!

So.. it seems I get the DISM all happy, but still get this weird error - Error in operation: (null) (CBS HRESULT=0x800f0831) when I go to try to apply any update.

No logs attached.
