It isn't every security update that fails. We deploy security only updates right now and will as needed deploy a quality roll up. The March security only update didn't install on this server but April and May did. I just tried to install the May Quality Rollup (4019215) and it failed.
Ran SFCFix and it says source files can't be downloaded. Contents of the log file are here:
