Server 2012 R2 updates install and after the restart, the same update reappears

Hello.

I have been having some issues with server updates where the update is installed. After the server restarts, it shows as restart pending in the update history and a few hours later, the same update is offered to be installed although it was successfully installed. Any help is much appreciated. I am attaching a screenshot.
