attached are the zip and an hour long Latency Monitor report

other things that i have tried
- Uninstall all recent software going back 30 days
- Updated Realtek audio drivers
- Disconnected all drives except boot drive

the issue persists and essentially makes my system completely unusable, so any help with this would be greatly appreciated.

Your default playback device is noted as: LG TV (NVIDIA High Definition Audio): (default)

This strikes me as odd, not only because it is identified as LG TV but also because you do have Realtek listed, but as secondary. I can't figure out how an LG TV driver would end up on here at all.

unfortunately due to a misshap with my monitor while moving i've been forced to use my TV as a screen for quite some time, so it's likely that a driver was pulled when my pc detected it as an output. It would also seem that realtek as a secondary is how i switch to using my headset, not sure if that's how it's supposed to be, but it's been that way for as long as i can remember where as this issue only cropped up today

Then it's as much a mystery to me as to you.

Are you certain that no Windows Updates have been applied very recently? There was an optional cumulative update over the last few days, but those don't install themselves - you have to activate the Download and install link in Settings, Update & Security, Windows Update pane.

Have a look at my pinned topic: Britechguy’s Standard “Windows 10 Revival Script”

And I'd actually start at looking at your update history to see if anything did get applied in a time frame where you are fairly certain was the "trigger point" for the issue.

Taking a look at update history it doesn't seem like there's actually been anything in a while and just now i see that there are two updates available plus another optional update, which is now making me feel like installing those may even fix the problems?

It appears that you have a regular cumulative update, KB4551762, that has failed to install 11 times, which is quite peculiar, and needs to be remedied.

Were I you, I would follow my instructions for doing a Repair Install using the ISO file, and you should download that now. Do not use an old 1909 ISO, even if you happen to have one previously downloaded.

You could do the SFC/DISM pair prior to trying the Repair Install, as that could also fix whatever it is that's a stumbling block for the previously noted cumulative update.
